Practical Training and Extracurricular Activities

The college provides practical training to LL.B. students, with compulsory participation in Moots along with court visits. Healthcare Initiatives and Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY)

Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) seeks to provide Universal Health Coverage by 2030, addressing the healthcare needs of India’s impoverished population. Despite its potential, significant gaps in enrolment and awareness persist, particularly in rural areas. A community-based sequential explanatory mixed-method study was conducted among 540 households over 18 months in the field practice villages under Rural Health Training Centre (RHTC) of a private medical college in Puducherry. The study participants comprised heads of households categorized under the Below Poverty Line (BPL) as per the Socioeconomic Caste Census 2011, and permanent residents of the selected villages.

Awareness and Utilization of AB-PMJAY

The survey revealed that 71.7% of the beneficiaries were aware that AB-PMJAY is a government-funded health insurance scheme, but only 33.5% knew it covers only BPL families. The utilization remained low at 2.03%. Additional strategies are required to enhance utilization among beneficiaries. In Puducherry, the scheme was initiated in August 2020, with 1.97 lakh families identified and 125 Common Service Centres established to facilitate e-card distribution. Despite nearly 500 million Indians being eligible for PMJAY, only 50% have enrolled, indicating substantial enrolment deficits, compounded by low awareness among those who require the benefits most.

Study Findings on AB-PMJAY in Rural Puducherry

The study design employed was a community-based explanatory sequential mixed-method design. A pretested semi-structured questionnaire consisting of 18 components was administered to gather quantitative data, assessing awareness, knowledge, enrolment status, and healthcare utilization under the AB-PMJAY scheme. The eligibility criteria for the selection of study participants comprised heads of households (HoH) categorized under the Below Poverty Line (BPL) as per the Socioeconomic Caste Census 2011, who were the permanent residents of the selected villages. In-depth interviews (IDI) were conducted to capture qualitative data regarding personal experiences, barriers to enrolment, and perceptions of the scheme.

Key Findings

Awareness: While 71.7% of study participants recognized PMJAY as a government-funded health insurance initiative, only 33.5% knew it specifically covers BPL families.
Utilization: Only 11 (2.03%) of the eligible beneficiaries had utilized the scheme.
Knowledge Scores: Age category (p = 0.006), educational status (p = 0.006), and occupation (p = 0.021) were found to be statistically significant predictors of knowledge scores, indicating that middle-aged individuals (40-60 years), those with higher education levels (graduates), and employed individuals with physically less demanding jobs demonstrated better knowledge about the scheme.

Qualitative Insights from In-Depth Interviews (IDIs)

Manual thematic analysis of IDIs resulted in three themes: facilitators, barriers, and suggested measures in improving PMJAY utilization.

Facilitators and Barriers: Scheme-related factors, patient-related factors, and hospital-related factors were the categories under the theme of facilitators and barriers for utilization of the PMJAY scheme.
Suggested Measures: Policy-related factors and patient-related factors were presented under the theme of suggested measures in improving the utilization of the PMJAY scheme.

One respondent stated, "With the help of this card only I got the surgery done at the earliest or otherwise this surgery would had imposed extra financial burden on me." Another respondent lamented, "I spent nearly 4 lakhs by borrowing money from my relatives to undergo an orthopaedic surgery in a private hospital. I would have utilized PMJAY card if I was aware of that the scheme at that time."

Comparison with Other Studies

The study found that the coverage of the PMJAY was 62.6%, and the knowledge levels among the beneficiaries were varied based on the socio-demographic factors such as age and literacy. The utilization rate of the PMJAY scheme in the present study was only 2.03%, which is significantly lower compared to studies conducted in Gujarat and Chennai.

Implications and Recommendations

The findings of this study indicate that while awareness of the AB-PMJAY exists among eligible beneficiaries in rural Puducherry, actual enrolment and utilization rates remain critically low. This suggests the need for sustained educational interventions and policy measures that address the systemic barriers preventing beneficiaries from fully accessing the scheme’s services. Integrating AB-PMJAY awareness into routine consultations at primary health centers could enhance enrolment and utilization.
