This form does not result in an immediate withdrawal. The withdrawal date will be the date of the form. A "W" grade will be issued, indicating withdrawal. Note that courses may not be withdrawn after the 80 percent mark of the course.

Deadlines may fall on Saturdays, Sundays, or holidays when offices are closed, so plan accordingly.
