The OF Charitable Grant is only available to AO member dental professionals. Patients cannot apply for this grant. Charitable grants will be determined based on three major requirements: Care providers, Patients, and Corporate partners.

Care Provider Requirements

The primary care provider in each grant application must be a member of the Academy of Osseointegration. The funds provided will be used for the care and treatment of only the individual (i.e. patient) specified in the application. These funds should subsidize all or part of the care needed by the patient, and a commitment to complete therapy and including a 3-year follow-up is implied and required when the provider accepts the grant funding. The patient should not incur any financial obligations. All team members must agree to relieve the patient of any financial responsibilities. Proposals will only be funded if the diagnosis and full treatment plan (including a restorative plan) for a particular patient is submitted for evaluation as part of the proposal. The identity of a patient is irrelevant to the evaluation of any proposal. Documentation, both pre- and post-operative, in the form of projection-quality slides and radiographs, is required for each grant-funded case. Patients may be asked to agree to be photographed or filmed for promotional activities by the Osseointegration Foundation. A patient release form will be required for this purpose. Three years of follow-up to the Osseointegration Foundation is required. Follow-up should be submitted to the Osseointegration Foundation on an annual basis.

Patient Eligibility Requirements

The patient must meet the eligibility requirements in each category: Physical disability â there must be the presence of a disability that is amenable to implant therapy and for which implant treatment would significantly improve the patientâs dental deficit and quality of life. Economic status â documents providing evidence of the patientâs inability to receive needed implant therapy because of economic constraints must be presented. Economic hardship, however, is not the primary or sole criterion for awarding a grant. Emotional well-being â the benefits of psychological or emotional improvement from implant therapy will be considered in the award selection process.

Corporate Partner Participation

The design of this program requires involvement of the Osseointegration Foundationâs (OF) commercial partners service and/or hardware for the patients who become grant-funded. All materials should be donated by the specified company. Grant applications must specify the particular products (including manufacturer names) at the time of grant application. Commercial entities should be contacted by the applicant to obtain donations for the required materials. If the applicant does not have a specific company or contact to obtain these donations, the OF will provide a list of potential suppliers. Both care providers and corporate partners may participate in this program for public relations purposes. Additionally, participants in the grant program, including care providers and corporate partners, will receive narrative recognition at all opportunities (i.e. at official Academy meetings), The International Journal of Oral and Maxillofacial Implants (JOMI), and the Osseointegration Foundation Report. This grant does not restrict how or where these funds are to be allocated by the service provider, as long as the entire grant is spent directly on patient care and treatment in the domain of osseointegration.

Additional Scholarships, Grants, and Loan Repayment Programs for Dental Students

The following is a non-exhaustive list of scholarships, grants and loan repayment programs available to dental students. The ICD-WUDAA Scholarship Program offers annual scholarships to dental students who demonstrate financial need and are in good standing at a USA dental school approved by ADA/CODA. The DDF evaluates each application on its own merits, and careful consideration is given to each request. A Cosmetic Dentistry Grant (CDG Grant) is free to apply for and is available to anyone that wishes to improve their oral health and return confidence to their smile. To be eligible for consideration of a CDG Grant, an applicant simply needs to demonstrate that their mouth is healthy enough to support the type of cosmetic dentistry procedure they wish to receive. The dentist will not compromise the function or health of your mouth to accommodate a cosmetic procedure. Accordingly, the dentist will advise in advance if you require any basic dentistry such as root canals, fillings, cleanings or extractions. If you do require any basic dentistry, you must complete it in advance of being recommended for consideration of a Cosmetic Dentistry Grant. It all starts with wanting to have your smile reflect the confident person you truly are. To determine your eligibility, you will be provided with a free oral health assessment by a Certified Dental Practitioner in your area. A potential grant recipientâs residential address must be within a 60-mile radius from a participating CDG dentist. Once becoming eligible for a partial grant, applicantâs must proceed with their cosmetic dentistry work with the dentist they were assigned to, or another participating dentist in the program. Notification of awarding of a partial grant will be 30 to 60 days following an applicantâs completion of all basic dentistry and becoming recommended for inclusion in the grant program. Once acknowledgement and acceptance of the grant is received within the 30 day acceptance period, applicants are entitled to initiate their treatment immediately with their participating CDG dentist. Payment of an applicantâs partial grant is made directly to the dental office and not to the applicant. To be eligible for consideration of a Cosmetic Dentistry Grant, you must receive a signed declaration form from the examining dentist confirming that you do not require any basic dentistry work such as fillings, cleaning, extractions or root canals. If basic dentistry is required, it must be completed at your own expense. Grant recipients must complete their cosmetic dentistry with the dentist that recommends them as a candidate for consideration of a cosmetic dentistry grant. Awarded recipients must begin their treatment plan within 3 months. The dentist determines eligibility.
