The Jamie Bax Memorial Scholarship: Empowering Students with Cystic Fibrosis

The Jamie Bax Memorial Scholarship is a valuable financial aid opportunity designed to support students diagnosed with Cystic Fibrosis (CF) as they pursue higher education in the United States. Created in loving memory of Jamie Bax, who bravely fought Cystic Fibrosis throughout her life, the scholarship aims to empower students living with CF by easing the financial burdens associated with college and helping them achieve academic and career success. Sadly, Jamie passed away in February 2019 at the age of 31. This scholarship serves as a tribute to Jamie's life and aims to honor her legacy by supporting individuals affected by Cystic Fibrosis in pursuing their educational goals.

Scholarship Overview

Beginning with the 2024-2025 school year, three scholarships will be awarded annually. The number and amount of the scholarships available will be dependent on annual funding.

  • One scholarship of $5,000
  • One scholarship of $3,000
  • One scholarship of $2,000

These scholarships are to be used for attending a college, university, or trade school while having Cystic Fibrosis. The funds are to be used for tuition, fees, books, school supplies, meal plans, or housing costs. All awards will be paid directly to the college, university, or trade school of choice. No award will be paid to the recipient.

There is no limit on the number of years a person may apply, but the scholarships must be reapplied for on an annual basis to be considered. An individual can only be awarded a maximum of four scholarships. No special consideration will be given to previous recipients.

Eligibility Criteria

Applicants must meet the following criteria:

  1. Cystic Fibrosis Diagnosis: Applicants must have Cystic Fibrosis. Provide a letter on medical center letterhead, signed by a licensed physician, nurse practitioner, physician assistant, or certified practitioner involved in your care.

  2. Citizenship: Applicants must be U.S. citizens.

  3. Residency: Applicants must be a resident of the Central United States. You must reside in one of the following states: Alabama, Arkansas, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Michigan, Minnesota, Mississippi, Missouri, Nebraska, North Dakota, Ohio, Oklahoma, South Dakota, Tennessee, Texas, West Virginia, or Wisconsin.

  4. Enrollment: Applicants must be enrolled in or accepted to an accredited junior college, college, university, or trade school. You must submit proof of admission, acceptance, or enrollment. (Incoming college freshmen must supply an official high school transcript and a copy of their acceptance letter or confirmation of enrollment from their college, university, or trade school. Current college or university students must supply an official transcript and confirmation of enrollment from their college, university, or trade school.)

  5. Academic Standing: Applicants must have a cumulative GPA of 2.70 on a 4.00 scale and must be in good academic standing and not currently on academic warning or probation.

  6. Photo: Applicants must submit a recent photo, in JPG format, of themselves for identification purposes.

Application Requirements

To apply for the Jamie Bax Memorial Scholarship, applicants must complete the following steps:

  1. Complete the Application Form: Fill out the entire form and write “N/A” where sections do not apply.

  2. Personal Essay: Write a 500-1,000 word essay addressing:

    • Your educational and professional goals
    • How Cystic Fibrosis has impacted your life
    • Personal achievements or challenges you have overcome

    The essay allows the Selection Committee to understand you in a broader, more conversational way. Please write in a clear and professional tone. Share any additional details about your background, experiences, or aspirations that strengthen your application.

  3. Letters of Recommendation: Applicant must provide THREE letters of recommendation:

    • One from a Healthcare Team Member. (Any physician, nurse practitioner, physician assistant, or any certified practitioner directly involved in treating your CF.)
    • One from a School Official. (Any employee of the school in which you are enrolled. This includes, but is not limited to, teachers, special-interest group leaders, or coaches.)
    • One from a Community Member. (Someone whom you know well - such as an athletic instructor, coach, pastor, or any other person you know from your community involvement such as your Boy Scout/Girl Scout leader.)

    In their letters, they should let the Selection Committee know what personal qualities, examples of achievements, or scenarios support their endorsement. What would make you a good candidate for this scholarship? They should base it on the time they have spent with you as your teacher, healthcare provider, or through whatever relationship they've had with you. Letters should be written on official letterhead where applicable (e.g., for healthcare or school officials).

  4. Required Documents:

    • Academic transcripts
    • Proof of enrollment or acceptance
    • Proof of Cystic Fibrosis diagnosis
  5. Optional Documents: FAFSA report or financial statement for demonstrating financial need. Applicants may provide documentation to support financial need, such as a FAFSA report or financial statement.

  6. Parent/Guardian Signature (if applicable): If the applicant is under 18, a parent or guardian’s signature is required.

Selection Criteria

The scholarship selection committee reviews applications carefully. Awards are primarily merit-based, but financial need is also considered. The committee seeks students living with Cystic Fibrosis who demonstrate academic dedication, perseverance, and community involvement. Our Selection Committee carefully evaluates applicants to award scholarships, and all decisions are final.

About Jamie Bax

The Jamie Bax Memorial Scholarship was established in loving memory of Jamie Bax. Jamie was sometimes referred to as an “Old Soul Trapped in a Young Person’s Body”. This description was taken from TheOdysseyOnline.com and really does describe what Jamie was like. "The old soul doesn't see the purpose of pursuing things that can be easily taken away from them. Additionally, old souls have little time and interest for the short-lived things in life, as they bring little meaning or long-lasting fulfillment for them."

Her parents, Chuck and Lynn Watson, created this scholarship with the aim of aiding college-bound students who have Cystic Fibrosis. JmeBax.org was established by Chuck and Lynn Watson in honor of their daughter, Jamie, who bravely fought Cystic Fibrosis until her unfortunate passing in February of 2019.
