Navigating Surgical Care with University Surgical Associates
University Surgical Associates (USA) is dedicated to providing excellent surgical care to the citizens of our region. Through the leadership of our physicians, in partnership with our employees, we strive to compassionately provide service to people. USA aims to provide access to quality surgical care to every individual. Today their network extends to more than 6 cities in the region with 30 skilled surgeons.
Insurance and Payment Policies
Understanding insurance coverage and payment policies is crucial for a smooth healthcare experience. USA accepts most major insurance plans. However, because each insurance plan is different, USA encourages you to become familiar with your insurance plan and network before seeking care. Although USA will often help you obtain pre-approval for operations as a courtesy to you, insurance companies place the responsibility on you, the patient, to verify if your procedure is covered. It is important to note that networks can change throughout the year.
Out-of-Network Considerations
If USA is considered out-of-network with your insurance provider, they will still provide your care; however, you will be responsible for any out-of-network fees and charges your insurance provider does not cover. Since many of our services are unique and are not provided elsewhere in the community, your insurance company may grant a waiver to allow us to care for you, even though we may not be listed in your plan.
Payments Due at Appointments
USA requires payment of all insurance deductibles or copays at the time of service. We require that all copay and applicable charges be paid at the time service is rendered. In the case of elective surgery, deductible and other applicable charges must be paid before elective surgery is performed. Remaining deductible charges will be billed and can be paid online. For your convenience, USA accepts payment at our registration desks. For your convenience, we accept Visa, MasterCard, Discover, American Express, cash, personal checks or cashier’s checks.
Payment Plan and Options
Fifteen (15) days following first notification of a balance owed, USA will send an update letter regarding the amount paid by their insurance carrier and balance owed by the patients. In the event a patient has no insurance and owes an outstanding balance, a letter will be sent detailing the balance owed (payable within 10 days) and several options to handle paying off the remaining balance, including a monthly payment arrangement. Plus, you’ll now be able to look up your balance and make payments or set up a payment plan all from your account.

To make your visit as convenient as possible, we offer online patient registration. No more filling out endless pieces of paper! From that link you can access the forms you need to complete. It is mostly check boxes to click and should take you less than 15 minutes to complete.
Community Support: Project Access
Project Access makes health care more available to low-income, uninsured residents of Hamilton County Tennessee. Project Access is coordinated by the Medical Society of Chattanooga & Hamilton County and the Medical Foundation.
