University of Utah MyChart: Your Secure Health Connection
MyChart at the University of Utah Health provides patients with personalized and secure online access to portions of their medical records, enabling them to manage and receive information about their health. It is a web-based application that you can access through a computer or mobile device connected to the Internet and an up-to-date browser, such as Edge, Chrome, Firefox, or Safari.
Accessing and Setting Up MyChart
Patients who want to use MyChart receive a MyChart activation code during their clinic visit. This code allows them to log in and create their own username and password. If you don't have an activation code, you can request one on the sign-up page. For your security, your activation code expires after 30 days and is no longer valid after the first time you use it.
If you forgot your password, you can contact the MyChart Patient Support Line at 210-358-4980 to request a new, secure password. If you’re having trouble logging in, click the “Forgot Username?” or “Forgot Password?” link below the login fields for assistance. You will go through two-step verification to verify your identity so you can recover your username or password.
Key Features and Benefits
MyChart offers a range of features designed to help you manage your health effectively:
- Access to Medical Records: View information on your health record, including prescriptions, lab results, allergies, and immunizations. Items that are part of your medical record include prescriptions, lab results, allergies and immunizations.
- Test Results: See most test results as soon as they become available, often at the same time as your provider. Your provider is able to determine which types of test results can be accessed through MyChart. Please give your provider time to review your results. He/she can put some of your results on hold until he/she is able to talk to you or if the information can cause harm or violate your privacy.
- Clinician's Notes: Access notes from your doctor or other healthcare providers.
- Appointments: Patients and proxies can schedule follow-up and same-day visits, screening mammograms, and flu shots (during flu season).
- Personal Information Updates: You can also send a request to update your record through MyChart that will be reviewed by your clinic.
- Secure Messaging: Communicate with your healthcare team about non-urgent health matters. Please note that MyChart messages should not be used for urgent situations.
- Medication Refills: Request prescription refills online. Medications that are more closely regulated by the FDA have different requirements for prescribers.
- Bill Payment: View and pay your medical bills online.
- Proxy Access: Allow a family member or personal representative to securely communicate with your doctor or view your record on your behalf. This communication is done using a MyChart account and is restricted to non-urgent matters.
Mobile Accessibility
Securely access your health information from your mobile device by downloading the MyChart mobile app. The MyChart app, designed by Epic Systems Corporation, provides patients instant access to their electronic medical records directly from an iPhone, iPad or iPod Touch. Once your account is established, you can download the MyChart application on your smartphone then, select University Health San Antonio. Once your identity is verified and associated with your smartphone, you will receive a link to accept.

Ensuring Privacy and Security
University of Utah Health takes great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, passwords and dual factor authentication if you choose to opt-in. You are in control of your password, and your account cannot be accessed without that password. You should not share your password with anyone.
MyChart is licensed to University Health from the Epic Systems Corporation and is fully compliant with federal and state laws pertaining to your privacy.
While logged into MyChart, if your keyboard remains idle for 10-15 minutes or more, you will be automatically logged out of MyChart to protect your privacy and information. We aim to protect your privacy and the security of your information.
Proxy Access Explained
Proxy access allows another person to manage a patient’s MyChart account. For children under 12, proxy access allows a parent or legal guardian to manage their MyChart account and health information. The person requesting proxy access must have a MyChart account. Once access is granted, your child’s medical record will appear in the proxy section of your MyChart account.
Adults over age 18 can share their health information with another person through proxy access. Both people must have a UH MyChart account. Proxy access does not make the proxy responsible for the bill. The guarantor is the person responsible for paying the bill. Only the patient can invite an additional proxy.

Proxy users can set their own communication preferences for notifications within UH MyChart by selecting the patient’s profile in UH MyChart after login. Visit UH MyChart account settings, then select the Communication Preferences option located within the UH MyChart Account Settings. In order to receive text notifications, the phone number on file must be opted into receiving text messages; be associated with the proxy’s account; and have communication preferences set to “Yes” for text messages.
Proxy access is granted once a proxy form has been completed and processed by the clinic where you are receiving care. Yes, proxy access may be given to court appointed legal guardians. In order to have access granted, you will need to provide a copy of the court order granting legal guardian access to the patient’s health information. Proxies have full access to a child’s chart until the child turns 18 years old.
Parents and legal guardians of minor patients (13-17) can complete consent form to allow their teen to establish a personal MyChart account with University Health. The personal MyChart account will enable your child to view appointments and access more health information than is available through standard proxy access. To obtain proxy access, you must first complete a Proxy Consent Form. This is not recommended. MyChart offers direct access to your personal health record, communicating about another individual's information would place their information in your health record.
Integrating with Other Healthcare Providers
If you’ve been seen at another healthcare organization, you might be able to view information from that medical record right in MyChart. You might have heard of this feature referred to as Happy Together. To view this information, you must link your account. In the search bar, enter the account you want to link. Once your account is linked, you can click on the tab titled Linked Accounts, found at the top of your screen, to see which and/or how many accounts you are linked to.
Addressing Common Concerns
Inaccurate Information: If you find inaccurate information in your MyChart record, ask your provider to correct it at your next clinic visit. Your MyChart information comes directly from your electronic medical record at your provider's office.
